Wees net bewus daarvan dat die persoon reeds 60 jaar oud of ouer moet wees en dat ons geen Alzheimer- of Dementia-pasiënte of pasiënte kan neem wat binneaarse terapie/drup of voedingsbuis moet kry nie.
Just be aware that the person must be 60 years old already or older and that we are unable to take any Alzheimer or Dementia patients or patients that require to have Intravenous therapy/Drip or feeding tube.
Ons word slegs gesubsidieer vir 22 beddens uit die 38 beskikbare, aansoekers sal op ‘n waglys gaan vir ‘n gesubsidieerde bed om oop te maak. Vorms moet voltooi word, ons kan slegs voltooide vorms wat ontvang is waglys.
U moet ook bewus wees dat indien ‘n Inwoner vir Staatsubsidie kwalifiseer, die verskil steeds deur die familie betaalbaar is. Die staatsubsidie is afhanklik van die gradering wat ‘n Inwoner op die keuringsertifikaat kry. Indien die persoon ‘n A2 is, kom die persoon in aanmerking vir R 984.00 subsidie per maand, indien die persoon ‘n A3 is, kom die persoon in aanmerking vir R 3,541 subsidie. A2 is semi-selfsorg en A3 is van ons afhanklik vir sorg. Ons neem die persoon se SASSA plus subsidie vir losies en dan betaal die familie die verskil.
We are only subsidized for 22 beds out of the 38 available, applicants will go on a waiting list for a subsidized bed to open. Forms must be completed, we can only waitlist completed forms that have been received.
You should also be aware that if a Resident qualifies for State subsidy, the difference is still payable by the family. The State subsidy is dependent on the rating a Resident gets on the Selection Certificate. If the person is an A2, the person is eligible for R 984.00 subsidy per month, if the person is an A3, the person is eligible for R 3,541 subsidy. A2 is semi self-caring and A3 is dependent on us for care. We take the person’s SASSA plus subsidy for lodgings and then the family pays the difference.

1. AANSOEK OM DIENSTE IN BADISA: Die vorm kan deur die aansoeker self of enige person ingevul word. Die aansoeker of gevolmagtigde moet die vorm teken.
2. GENEESKUNDIGE VERSLAG VIR TOELATING TOT TEHUIS/VERSWAKTE AFDELING: Die vorm word deur u Geneesheer ingevul en geteken
3. VERSLAG VAN MAATSKAPLIKE WERKER: Die vorm word deur u Predikant of Maatskaplike werker ingevul en geteken.
4. FINANSIËLE VERKLARING:
FINANSIËLE VERKLARING: KLIËNT of FINANSIËLE VERKLARING: GEVOLMAGTIGDE/BORG
a. Die vorm word deur die person wat by die bank werksaam is of uself ingevul.
b. Indien u ‘n pensioenaris is word u pensioennommer benodig.
c. Indien u ‘n belegging of beleggings het, word die kapitale bedrag sowel as rente wat u ontvang op die vorm ingevul en die bank moet vir u die bewys daarvan aanheg.
d. Indien u vaste eiendom het, word ‘n waardasie van die eindom benodig asook die belasting en versekering wat u betaal. Bewyse wat u van die munisipaliteit kan verkry, asook van u versekeringsmaatskappy, moet aangeheg word.
e. Heg asseblief die nodige bewyse aan, om u aansoek te bespoedig.
f. Die vorm moet geteken word deur die aansoeker en beëdig word by die Polisiestasie, Bankbestuurder, Predikant of enige Kommissaris van Ede.
The forms are filled in as follows:
1. APPLICATION TO SERVICES IN BADISA FACILITY FOR OLDER PERSONS: This may be filled out by the applicant themselves or any other person. The applicant or an authorized person must sign this report.
2. MEDICAL REPORT FOR ADMISSION TO HOME IMPAIRED DIVISION: This report is filled out by your medical practitioner.
3. REPORT OF SOCIAL WORKER: This report must be filled in by a Minister or a Social Worker.
4. FINANCIAL DECLARATION:
FINANCIAL DECLARATION CLIENT or FINANCIAL DECLARATION PROXY/SPONSOR
a. This report is filled out by someone at the bank or the applicant himself.
b. If you are a pensioner, your pension number is required.
c. If you have any investments, the capital amount plus the monthly interest is required on the report and bank statements as proof must be attached.
d. If you own any fixed property, a municipal assessment is required as well as all tax and insurance that is paid. All proof of the municipality and insurance brokers required.
e. Please attach all necessary documents and proofs to speed up your application.
f. The statement of income must be signed by the applicant and be certified at the Police Station, Bank Manager, Minister or any other Commissioner of Oaths.
